Please print out the appropriate forms below and fill them in before you leave for your vacation. Fax the forms and a copy of your certification card to +1 815 550-2237. Don't forget to bring them along with your C-card on your first diving day with us.
All Divers
Each diver must complete one of these two forms:
Liability release (for divers who will not be using nitrox)
or Liability release (for divers who will be using nitrox)
plus both of these forms:- Standard safe diving practices
- Medical statement
Young Divers
Are you diving with a minor? Fill out the three forms above for him or her, plus this one: Youth diving responsibility
Equipment Rental
Do you need rental equipment? Fill out the three forms above plus this one: Equipment rental agreement
